This is probably simplest articulation of the causes of poor decision making. While most of of it are well known factors, but I like the way author has put it. To summarize they are
Laziness
Not anticipating unexpected events
Indecisiveness
Remaining locked in past
Having no strategic alignment
Over-dependence
Isolation
Lack of technical depth
Failure to communicate
